3/12/2023 EnchantaVaughn Approved Behavior: When QMHP arrived, client presented positive behaviors in the home, as he prepared for the session to begin. Mom sat nearby to give his support when needed. Mother reports client needs assistance on being able to learn how manage stressors and implement coping skills. Behavior: During the next part of the session, client continues to need assistance with being able to implement problem-solving skills at his age to build healthy relationships in the home and school setting. Intervention: QMHP challenged client to identify any situation he has experienced that makes him feel defeated or misunderstood. QMHP isolated client from distractions to decrease disruptive behaviors and to reduce episodes. QMHP further explained different situations that can cause anxiety. QMHP provided feedback regarding client inquiries. QMHP continued to elaborate on ways client can cope and manage stressors. QMHP processed client positive behaviors. QMHP assisted client in developing more social skills in the home so that he can build healthier relationships with his sister and brother. QMHP provided positive feedback while assisting client in normalizing the changes that occur in the home. Intervention: QMHP had one on one with client to reflect on his evening. QMHP reflected on client goals and objectives to remind client oh his daily expectations. QMHP facilitated a discussion to explore client current feelings. QMHP praised client for his cooperation and managing his behaviors as best as he could. QMHP thanked client for trying and working on a part of himself that he stayed away from for so long. QMHP monitored progress in demonstrating individualized pro-social skills (e.g. anger management, education, problem-solving, conflict resolution, appropriate verbalization of feelings, and cognitive-behavioral programming). QMHP prompted client to prepare for tomorrow and remember to go to bed on time so that he is ready for school. Response: Client informed QMHP that he was happy and excited to come to session. Client said he enjoyed being around people that treat him like family. Client understands that in order to do his best, he has to feel his best. Client displays appropriate attitude during the session, indicating he was learning and having fun. Client loved expressing himself around his mom and QMHP, as demonstrated by his high verbalizations and commitment with answering different questions. Client made eye contact towards the QMHP, as he smiled with satisfaction and confidence. Client was monitored complying with the QMHP’s positive and empathetic acceptance. Client was open to the mutual and trusting relationship that was reestablished by the QMHP. Client is observed making better choices and doing the right thing. Response: Client reviewed last week discussion with psycho educational and retained a lot of the information he was taught which motivates him to pay more attention and take his time. Client is observed problem solving on his own and handling his emotions appropriately. Client appeared to have a clear knowledge of the interventions. Client displayed appropriate understanding of exercise. Client was sociable and friendly in regard to the QMHP’s positive empathy. Client was able to express and explore his perceptions of relationship skill building and this helped him in all areas of his life. Client was compliant in today’s session, as evidenced by him following directions. Client has presented himself in a responsible way and is making progress towards goals and objectives. Mom was satisfied and content in reference to the next session.
